Mindful Transitions Plan

Students will learn to identify and manage emotions related to change through mindfulness practices, group discussions, and reflective writing, setting personal intentions for future transitions.

Helping students navigate emotional challenges during transitions promotes resilience, self-awareness, and a positive outlook, supporting mental well-being and successful adaptation to new phases.

Mindful Transitions: Letting Go and Looking Forward

Welcome students. Introduce yourself and the lesson title. Explain: “Today we’ll explore the emotions that come with change, practice mindfulness together, and set positive intentions for transitions ahead.”

Understanding Transitions

Briefly define transitions and invite 2–3 students to share one recent change they’ve experienced (e.g., moving to a new grade, a new school).

Guided Mindfulness: Breathing

Guide students through the breathing exercise slowly. Model each step and remind them to focus on the breath and let thoughts pass without judgment.

Guided Mindfulness: Body Scan

Lead the body scan: start at the feet and move up. Pause at each area, naming sensations. Emphasize curiosity, not judgment.

Group Discussion

Divide the class into small groups or discuss as a circle. Encourage active listening and respect. Use additional prompts from the [Transition Discussion Guide](#mindful-transitions-discussion-guide) as needed.

Individual Reflection

Distribute the [Mindful Reflection Worksheet](#mindful-transitions-reflection-worksheet). Circulate to offer support or clarify prompts.

Setting Intentions

Encourage students to be specific and realistic. Offer examples: “I intend to practice 5-minute breathing each morning,” or “I will ask a friend how they’re doing when I feel anxious.”

Closure and Next Steps

Invite 2–3 volunteers to share one intention. Remind students that mindfulness is a skill they can revisit anytime. Thank them for their openness and participation.
